Symptoms:

Feels like a slipping clutch

Abnormal noises from transmission/clutch area

Gear engagement problems

Possible Damage:

Grease / oil / fluid on pressure plate

Heat damage to pressure plate surface

Gouging to pressure plate surface

Broken / damaged pressure plate fingers

A damaged pressure plate is sometimes hard to diagnose, as it may feel like just a clutch failure.  Symptoms include a slipping clutch, abnormal transmission/clutch noises, or tranmission gear engagement problems.  The only way to verify it's the pressure plate is to access the clutch system by removing the transmission or engine.
